Therapydia is an network of Physical Therapy clinics providing quality physical therapy and wellness services.

Your privacy and the security of the Personal Information you provide on our Site are our highest priorities. We use industry standard security and encryption with hosting redundancy at two locations in the US to provide continuous access to our service and to protect your Personal Information.

1. Personal Information Collection

Publically Available Information Collected

Therapydia has built a database of Physical Therapist information from publically available databases to provide a directory of therapists and facilitate connections between therapists. When registering for the site you will verify the information that we have collected and correct any information that might be out of date while completing your profile.

Registration Information

To become a member of Therapydia, you will search for and claim your pre-existing Therapydia profile, or you can create a profile by entering the required information: first name, last name, city, state, zip code, email address and password. Your email address will not be displayed publically on the Site and we will use it only to send you notifications and information you request. You can opt to not to receive these updates at any time.

The following optional information may also be entered to complete your Therapydia profile: address, occupation, specialty, degree type, states of professional licensure, workplace information, college you received your professional degree from and year of graduation, insurance providers accepted, biography, blog information, and social account information. Some of this information will already be included in your profile but you will be asked to verify that the information is correct, and update it where appropriate.

All of the information that you voluntarily provide will be included in your profile, which is accessible to anyone browsing the Therapydia site, except for your email address and password.

Right to Access and Modifying Your Personal Information

You have a right to access, modify, correct and eliminate the data about you that has been collected pursuant to your decision to become a User. 2. Personal Information Use

Therapydia is a Physical Therapy community, enabling PTs to connect with one another. The information you choose to provide on our service is used to help you describe yourself to other Users and site visitors. Other information that does not personally identify you as an individual is collected by Therapydia from Users (such as, for example, patterns of utilization) and is exclusively owned by Therapydia. This information can be utilized by Therapydia in such manner as Therapydia, in its sole discretion, deems appropriate.

Site Usage Tracking

Therapydia uses cookies and web log files to track site usage in the same way most sites do. A cookie is a small data file that resides on your computer, and is accessed through the browser, which allows Therapydia to recognize you as a User when you return to the Therapydia website using the same computer and web browser. Like the information you enter at registration or when you update your Profile, cookie and log file data may be used to customize your experience on our website. A “persistent” cookie is set once you’ve logged in to your Therapydia account. The next time you visit the Therapydia website, and for 30 days thereafter, your persistent cookie will allow us to recognize you as a member so that you will not need to log in before using the service. However, if you log out of the Therapydia website you will need to sign in again with your email address and password the next time you visit in order to log in to your account. We also use cookies to improve the quality of our service by storing User preferences and customizing your experience accordingly. You can set your browser to refuse all cookies or to indicate when a cookie is being sent if you prefer.

Log Files, IP Addresses and Device Information

When you visit the Therapydia website from another site we automatically receive the URL of the site from which you came and the site to which you are going when you leave Therapydia. Therapydia also receives the internet protocol (“IP”) address of your computer (or the proxy server you use to access the World Wide Web), your computer operating system and type of web browser you are using, email patterns, mobile device operating system (if you are accessing Therapydia using a mobile device), as well as the name of your ISP or your mobile carrier. We use this information to analyze overall trends to help improve the service. The linkage between your IP address and your personally identifiable information is not shared with any third parties.

Consent to Therapydia to Use the Information You Provide Us

Certain information you provide to Therapydia may reveal, or allow others to identify, your nationality, ethnic origin, religion or other aspects of your private life, and more generally about you. Please be aware that in providing information to Therapydia for the purposes of opening your account, you are expressly and voluntarily accepting the terms and conditions of this Privacy Policy and Therapydia’s User Agreement. The supplying of all such information by you to Therapydia, including all information deemed “sensitive” by applicable law, is entirely voluntary on your part. You have the right to withdraw your consent at any time, in accordance with the terms of this Privacy Policy and the User Agreement, but please note that your withdrawal of consent will not be retroactive.

Therapydia’s Communications to You

Therapydia will communicate with you through email and notices posted on the Therapydia website. Our communications to you include emails that help inform Users about various features of the service. You can change your email and contact preferences at any time by updating your profile information.

Sharing Information with Third Parties

Therapydia takes the privacy of Users very seriously. We do not sell, rent, or otherwise provide your private information to third parties for marketing or other purposes. We may provide aggregated anonymous data about the usage of our services to third parties for such purposes as we deem, in our sole discretion, to be appropriate, including to prospective investors in Therapydia. We may segment our Users by specialty or geographic location. We may aggregate or provide information from your public profile. If you would like to be excluded from the aggregated research or products based upon aggregated data about our Users’ activities on the site, please email [email protected].

Finding Colleagues

We use your information, including the addresses you import through our contact importers, to make suggestions to you and other users on Therapydia. To help your friends find you, we allow other users to use contact information they have about you, such as your email address, to find you, including through contact importers.

Search

We offer search services to help you find information we have collected about you and to and learn more about other Users (for example, you can search for Users with particular expertise, or that you may know from college).

Legal Disclaimer

It is possible that we may need to disclose Personal Information when required by law, such as responses to civil or criminal subpoenas, or other requests by law enforcement personnel. We will disclose such information when we have a good-faith belief that it is necessary to comply with a court order, ongoing judicial proceeding, subpoena, or other legal process or request to Therapydia brought in any country throughout the world, or to exercise our legal rights or defend against legal claims.

Disclosures to Others

We may also disclose your personal and other information you provide, to another third party as part of a reorganization or a sale of the assets of Therapydia. Any third party to which we transfer or sell Therapydia’s assets will have the right to continue to use the personal and other information that you provide to us.

 

3. Personal Information Control

Accessing and Changing Your Account Information

You can review the Personal Information you provided us and make any desired changes to the information you publish, or to the settings for your Therapydia account, including your email and contact preferences, at any time by emailing [email protected]. You can also make these changes on the Therapydia website by updating your Profile.

Please be aware that even after your request for a change is processed, Therapydia may, for a time, retain residual information about you in its backup and/or archival copies of its database.

Newsletter Opt-Out

You may choose to stop receiving Therapydia Newsletters by following the unsubscribe instructions included in these emails or you can contact us at [email protected].

Closing Your Account

If you decide to close your account for any reason, just email us at [email protected]. We will then remove all of your information from the website. Your data will continue to reside in our database including data contributed by you on your profile and any data we believe may be important to prevent fraud or future abuse, or for legitimate business purposes, such as analysis of aggregated, non-personally identifiable data, or account recovery.

 

4. Your Obligations

As a User, you have certain obligations toward the other Users with whom you will be communicating. Certain of these obligations are imposed by applicable law and regulations, and others have become commonplace in user-friendly communities of like-minded members, such as Therapydia:

  • You must, at all times, respect the terms and conditions of the then-current Privacy Policy and the User Agreement. This includes respecting all intellectual property rights that may belong to third parties.

  • You must not download or otherwise disseminate any information that may be deemed to be injurious, defamatory, violent, offensive, racist, sexist or xenophobic, or which may otherwise violate the purpose and spirit of Therapydia and its community of Users.

  • You must not provide information to Therapydia and /or other Users that you believe might be injurious or detrimental to your person, professional or social status.

  • Any violation of these guidelines may lead to the restriction, suspension or termination of your account by Therapydia, as we take these principles seriously and consider them to be the basis on which our Users adhere to the Therapydia website and the services which it offers.

 

5. Important Information

Insurance Data

Insurance data on Therapydia is for informational purposes only, should not be relied upon, and users agree to hold harmless Therapydia and it’s data suppliers for use of the data.

Changes to this Privacy Policy

Please review this privacy policy frequently as we will modify it at any time. If we make material changes to this policy, we will notify you here, or by email. By continuing to use the Therapydia service after notice of changes have been sent to you or published on the Therapydia website, you are consenting to the changes.

6. Security

Insurance Data

In order to secure your Personal Information, access to your data on Therapydia is password-protected. It is your responsibility to protect the security of your login information.

We use secure technology, privacy protection controls, and restrictions on employee access in order to safeguard your Personal Information.

We use a secure, respected hosting provider to store and serve our site and data. We use firewalls and other access controls on our servers that are monitored to help protect against security vulnerabilities.

Each and every one of our employees servicing our sites are obligated to abide by this Privacy Policy. Only authorized employees servicing our sites are permitted to have access to your Personal Information, and such access is limited by need.

Since the Internet is not a 100% secure environment, we cannot ensure or warrant the security of any information you transmit to Therapydia. There is no guarantee that the information may not be accessed, copied, disclosed, altered, or destroyed by breach of any of our physical, technical, or managerial safeguards.

Any improper collection or misuse by unauthorized third parties of personally information, or other information provided on the site is a violation of the terms of use and should be reported to [email protected].

Jessica Jones

Physical Therapist

Jessica recently moved to Seattle from Boston, MA and is excited to join the Therapydia team. Her treatment experience includes orthopedics, sports medicine, pediatrics, and vestibular therapy. Jessica received her Doctorate in Physical Therapy from Northeastern University in Boston, MA in 2016. She has completed the Pediatric Physical Therapy Residency at Boston Children’s Hospital and is in the process of completing a Comprehensive Vestibular Rehabilitation certification. Jessica believes in empowering and inspiring patients to take control of their health through education, movement, and exercise. She enjoys treating patients of all ages and levels while utilizing soft tissue techniques, neuromuscular re-education, balance training as well as therapeutic exercise. In her free time, Jessica enjoys yoga, dancing, kayaking, and hiking with her dog!
